MOH Exam Registration Detailed Procedure

Step 1: Documents to be required
a. Pharm D (Degree)
b. Experience Certificate (Minimum 2 years – only retail and hospital would be acceptable – Experience may also be counted after degree issuance date)

c. Good Standing Certificate from regional Pharmacy Counsel (Take picture before seal)
d. License (Category A)
e. HSSC & SSC (Degree)


Step 2: Go to https://smartforms.moh.gov.ae:444/SignUp.aspx make account and login.

Step 3: Add required details. Name, profession, education, experience and license.
Tip: 
1: Select Pharmacist, General Practitioner and Pharmacy for Category, Subcategory and speciality respectively from dropdown options. (you may also confirm about the option, which one to select)
2: For Arabic name just go to google translator (English to Arabic) write required name there and get Arabic word. Copy and paste it.


Step 4: Upload all documents mentioned above including F.Sc and matric. Also upload good standing certificate in education field. 
Tip: Use CamScanner (android application) for scanning the documents to upload.


Step 5: From home screen (after selecting evaluation) make payment by clicking on make payment option. Add Bank account details and pay online. It would be account activation fee. 258.06 AED = 7800 (exclusive of bank transfer fee). 
Tip: Use allied account for payment process. You need to have Allied Bank account and online internet banking activated. Call Allied helpline to activate online banking session for your debit card. Add details on the site and pay fee. Allied do not charge much for the payment. Only 200 for each transaction. For first payment 8000 will be deducted (according to current currency rate).


Step 6: After the payment of 258.06 AED you account will be activated, and you get the option to submit the application. Check the documents again and submit the application.

Step 7: After submission you will get a mail from DataFlow team, they provide the link for payment. They will verify your documents (Pharm D, Experience Cert., Good Standing Cert., License). You need to pay 1076 AED (approximately 33000 RS if you pay from above method of payment using allied). They took 1 month (maximum) to verify your documents. You can also track your application via given details by DataFlow team.

Step 8: After getting DataFlow report you are eligible to schedule your test date and center. Fee for this step would be around 15000-20000 RS.
